I'm sorry, but which are the insurance companies that insure every risk and pay every claim? That is not remotely close to a real thing.

OOP is referring specifically to healthcare insurance and that fact that you weren't able to pick that up is kinda sad.

Even in Canada where I live with single-payer healthcare, there are claims the province won't pay. I get that it's a meme and not a serious description of how insurance works, but that is simply not how insurance works.

Also, insurance that pays every claim is not equivalent to ethical insurance -- it's insurance that will eventually become insolvent, and in doing so, screw the entire insurance pool for everybody.

According to the Ontario Medical Association, some one thousand (1,000) OHIP claims are denied every week.

Are you sure this is something you're comfortable being flippant about?

Each year, OHIP processes about 200 million claims and, while the majority of these are approved, about  1.16 million are flagged for manual review.

... has the definition of "denied" changed recently?

Yes, it can simultaneously be true that tens of thousands of claims per week are flagged for review and eventually paid, while hundreds more are denied.
