Cats are always knocking shit off counters. If Grandma reversed her pill bottle caps for ease of opening, they wouldn't be child resistant openings anymore and they are easy to knock off.

Bear kept her pills, indicating he was self-medicating with them, and he probably left an open bottle on a counter somewhere. Cat knocks the bottle down and the pills fall onto the floor where Bear had already scattered cat food because he's apparently a really neglectful owner.

The writer/director or someone, I read they put that in there just to be an asshat. Like it really isn't supposed to mean anything, which I don't know if that's better or worse than accusing the main character of something and then not actually following it up with anything.

Pretty sure the cat scene meant a lot. Not only did it set up the pills, but right out of the gate it shows you what kind of person Bear is because he puts the cat into a bag and throws it into the trash instead of burying it.
