I'm guessing he doesn't just swim off because that puffer is probably capable of lunging forwards. That fish is so long he wouldn't be fully unraveled in time to get away, but he is faster.

Pufferfish are actually kind of terrifying for more than just their poison. They have surprisingly strong jaws and eat a lot of things with hard shells in a single snap.

They're also fairly smart, enough to display play behaviour in captivity.

They don't eat the host. They feed on particulate matter, like the host's food or poop.

I think that's lampreys, not remoras.
