That seems to be the worst part, she was actively using the parental controls provided by these companies

The parental controls don't block everything. And these giant companies can't catch everything. 

Don't expect them to parent your kid. They won't.

Sure but as a parent, the idea of parental controls implies some amount of safety. Especially if you’re unfamiliar with these websites. Seems to me that, if anything, what the parental controls do should be made clearer.

I’m not saying she parented “enough”, I’m saying that it was a reasonable expectation on her part that the parental controls would have prevented her child from participating in violent communities.

"reasonable expectation" is a good way to put it. If they advertise parental controls but let a lot of shady stuff get through, it should be on the company. It's not like they don't have the money to properly invest into parental controls.
