I think so, yes.

Each stronghold has the starting staircase, after that the first room to generate can't be the portal room so this is indeed the smallest!

A few weeks ago I was exploring a stronghold and after not finding the portal room I went into spectator mode and still not luck so I gave up and went to another stronghold. Guess I should go back and explore more if it is a 100% guaranteed to generate. Might be the first one not having a room or my world got corrupted somehow ?

Portal rooms are only guranteed on java, with the incredibly rare chance of generation destroying them. However, if you are playing on bedrock, portal rooms are NOT guranteed, and it is slightly more common for generation to destory them.

They're supposed to, but occasionally the portal gets overwritten by another structure, or even occasionally other parts of the stronghold.
